Dr. "Z" has been in the community for many years and now is restricting her practice to gynecology. She and her medical assistant Sharon, will be in the office 3 days a week. | | GOODBYE DR "A" Dr. Anderson retired on March 1, 2008, after serving the Kirkland area for 27 years. Medical records of patients who have been seen by Dr. Anderson within the past 2 years will be maintained in the office. If you have been a patient of Dr. Anderson's we would be happy to schedule your next appointment within our office. If you have moved away, or wish to have your medical record transferred to a different practice, please complete a transfer of records request and allow at least 2 weeks for copying and transmitting. | | INSURANCE CARDS & BILLING: We are happy to process your bill through your insurance carrier if we are contracted with them. To do so, we MUST have a copy of your insurance card. Patients without proof of insurance at the time of their appointment have 3 options. You may pay for your visit by cash or credit card and handle reimbursement from your insurance company on your own; you may call and obtain proof of insurance to be faxed to us from your insurance company, employer or family member who has a copy of your card; or you may reschedule your appointment for another time. |
